Our programs are intentionally designed to work together. Whether someone enters through healthcare, navigation, or community referral, they receive a single coordinated plan that flexes to their needs and goals over time.

Program 01

Housing Navigation

One-on-one navigation that helps individuals and families identify, apply for, and secure suitable housing options across the region.

Population served: Adults and families experiencing or at imminent risk of homelessness.
Desired outcomes: Decreased time from intake to housing placement and increased housing match quality.
Community impact: Reduces emergency service utilization and accelerates pathways to stability.

Program 02

Housing Stabilization

Post-placement supports (lease education, landlord mediation, financial coaching) designed to help participants remain housed.

Population served: Newly housed individuals and families in the first 12–24 months of tenancy.
Desired outcomes: High 12-month housing retention and reduced returns to homelessness.
Community impact: Protects the public and philanthropic investment behind every housing placement.

Program 03

Case Management

Dedicated case managers coordinate benefits, healthcare, employment, and community supports tailored to each participant's goals.

Population served: Program participants across all housing service lines.
Desired outcomes: Improved benefit enrollment, healthcare engagement, and self-defined goal progress.
Community impact: Connects fragmented systems into one coordinated plan of care.

Program 04

Recuperative Care Partnerships

In partnership with healthcare systems, we support medically vulnerable individuals discharging from hospitals into stable, supportive environments.

Population served: Adults discharged from inpatient care without safe housing to return to.
Desired outcomes: Reduced 30- and 90-day readmissions and improved post-discharge recovery.
Community impact: Lowers avoidable healthcare costs while improving patient outcomes.

Program 05

Community Resource Coordination

We coordinate across food, transportation, behavioral health, legal, and workforce providers to wrap services around each household.

Population served: Participants needing multi-domain support to maintain stability.
Desired outcomes: Faster service connection times and fewer unmet needs reported by participants.
Community impact: Strengthens the broader safety net by aligning community resources.

Program 06

Independent Living Support

Skills-based coaching in budgeting, tenancy, household management, and community engagement to sustain independent living long-term.

Population served: Participants transitioning toward fully independent housing.
Desired outcomes: Higher independence scores and sustained tenancy beyond program exit.
Community impact: Translates short-term stability into long-term self-sufficiency.

Program 07

Housing Readiness Services

Pre-placement supports that prepare participants for successful tenancy: documentation, rental readiness, and landlord engagement.

Population served: Individuals and families preparing to enter permanent housing.
Desired outcomes: Increased approvals on rental applications and reduced placement disruptions.
Community impact: Expands housing access for people often screened out of traditional markets.
