Housing Navigation

One-on-one navigation that helps individuals and families identify, apply for, and secure suitable housing options across the region.

Housing Stabilization

Post-placement supports (lease education, landlord mediation, financial coaching) designed to help participants remain housed.

Program 03

Case Management

Dedicated case managers coordinate benefits, healthcare, employment, and community supports tailored to each participant's goals.

Program 04

Recuperative Care Partnerships

In partnership with healthcare systems, we support medically vulnerable individuals discharging from hospitals into stable, supportive environments.

Adults discharged from inpatient care without safe housing to return to.

Reduced 30- and 90-day readmissions and improved post-discharge recovery.

Program 05

Community Resource Coordination

We coordinate across food, transportation, behavioral health, legal, and workforce providers to wrap services around each household.

Independent Living Support

Skills-based coaching in budgeting, tenancy, household management, and community engagement to sustain independent living long-term.

Housing Readiness Services

Pre-placement supports that prepare participants for successful tenancy: documentation, rental readiness, and landlord engagement.
